Orthodontists have long debated whether malocclusion can or should be treated with or without the extraction of permanent teeth. Nowadays the debate continues without any sign or expectation that the issue will be soon resolved. The presentation analyses seven key aspects of the topic, supporting evidence derived from clinical experience with data from the literature. Some fundamental aspects of dental, skeletal and aesthetic diagnosis are analyzed, emphasizing how the choice between an extraction or non-extraction treatment is multifactorial and must consider several aspects at the same time. Particular attention is also paid to less considered aspects such as periodontal support and long-term stability. Some clinical cases will be shown to better explain how our choice can have long-term consequences. Finally, tips on how to communicate the proposal of an extraction treatment to the patient are provided. Is the proposal for an extraction treatment detrimental to our business?
